We are pleased that you have chosen Milan Eye Center and our Atlanta ophthalmology experts as your eye care specialist. Our mission is to provide the most up to date medical care with compassion and caring.

Once you arrive at the offices of Milan Eye Center, you will be greeted by a member of our check-in staff who will ask you to fill out a patient questionnaire or you may fill out the necessary forms by clicking here. If you are a returning patient, we will be asking you to update our records with any changes in address, phone number, or insurance coverage. This is important as we strive to keep our files as current as possible. You will also be asked to provide us with your insurance ID cards, as we keep copies of these as part of your chart.

Milan Eye Center gladly accepts most insurances and health plans. Be sure to bring your insurance card with you every time you visit Milan Eye Center. Please notify the front desk whenever your insurance coverage changes so the medical staff can accurately maintain your important medical records.

Have questions about insurance? Contact Milan Eye Center if you want additional information and details pertaining to your specific insurance coverage. Many insurance plans require patients to make a co-payment at the time of their visit. Milan Eye Center makes every effort to make each patient’s visit as easy and time efficient as possible.

Flexible Spending Accounts & Tax Savings Accounts

Flexible spending accounts (FSA’s), (also known as flex plan, cafeteria plan, reimbursement account, flex 125, Section 125, tax savings plan, or medical spending account) are offered by many employers to offset medical expenses. This employee-sponsored program allows for pre-tax dollars to be set aside for medical costs, including LASIK. You may be able to have surgery virtually tax-free.

Tax Deductible Laser Vision Correction
