To setup a new Patient Portal account please contact a Milan Eye Center representative at 678-809-1138. Our team member will set up the patient profile in our system, and provide you with a “Patient Portal Key.”

How can I access the Patient Portal?

First, please contact a Milan Eye Center representative at 678.381.2020. Our team member will set up the patient profile in our system, and provide you with a “Patient Portal Key.” The following steps will help you set up your account.

Step 1: Go to the Milan Eye Center Patient Portal

Step 2: Select First Time Patient Portal Access

Leave the fields at the top of the page blank. Under “Please Select – I Am,” select the “First Time Patient Portal Access” option and then click “Continue.”

Step 3: Verify Your Details

Enter the patient details just as they were provided to your Milan Eye Center representative. Enter the Temporary Key that was provided by your Milan Eye Center Representative.

Step 4: Create Your User ID and Password

Enter the patient details just as they were provided to your Milan Eye Center representative. Enter the Temporary Key that was provided by your Milan Eye Center Representative.

Step 5: Login to the Patient Portal

Once your profile is created, you will be asked to log into the Patient Portal with your username and password. Select “Patient,” if you are the patient, or select “Authorized Patient Representative” if you are the patient’s legal caregiver or guardian.

Who should I contact if I am experiencing issues logging in to the portal?

If you’re having trouble with the Patient Portal, please contact a Milan Eye Center representative at 678.381.2020. Our team member will gladly help you troubleshoot any issues you are experiencing.

What can I do with Patient Portal?

– Demographics: Upload and update your demographic information, (ie; date of birth, address, phone number, email, emergency contact, etc.)

– Medical Information: Save time at your next appointment! Input your Allergies, Current Medications, Prior Surgeries, and Medical History.

– Forms: Save (even more) time at your next appointment. Read through and sign your consent forms prior to your next appointment. Don’t forget to click “Save and Print” every time you sign a consent form.

– Diagnosis & Care Plans: Find a summary of your recent appointments and the recommended care plan submitted by your doctor!

– Practice Messages: Send a message to our Patient Coordinator!

Can I set up an appointment with the Patient Portal?

This is a goal for the very near future; however, we are not accepting appointments through the Patient Portal at this time. To set up an appointment, please call 678.381.2020.

When I send a message through the portal, who does it go to?

Your message will go to a team member at Milan Eye Center. They will address your needs and respond through the Patient Portal. Please allow two business days for a team member to respond to your request.

How do I know if there is a message waiting for me in the portal?

Login to your patient portal. When there is a message you will see a red notification next to the “Practice Messages” tab. Click this to view the message.

What if I have an emergency? Should I use the messaging system on Patient Portal?

In the case of a medical emergency, dial 9-1-1 or visit the closest emergency room.

I forgot my password, what should I do?

There is a “Forgot Password” option in the bottom right corner of the Patient Portal login screen. You can select this button, input your information, and answer your security question to gain access to your account. From there, you will update your password.

How do I change my password?

After logging in, you can update your password by selecting “change password” in the top left corner under your name.
